Dear Valued Patient, 

As we begin a new chapter in 2025, Pointe Primary Care is focused on restructuring, rebranding, and renewing our commitment to the community and the patients we serve. We are reaching out to share some important updates about our practice.

Provider Transitions:

• Jennifer Shade, AGNP, transitioned to Atlantic Family Physicians on December 31st, 2024, and is now located at 2 Lee Ave #103, Georgetown, DE 19947. She can be reached at 302.856.4092

• Dr. Brian Prigg, PA-C, will be retiring from Pointe Primary Care on February 28, 2025. We extend our heartfelt gratitude for his years of dedication and service to both the practice and the community.

• Jennifer Willey, FNP, will be leaving Pointe Primary Care on February 14, 2025, to join Christiana Care at Halpern Plaza, located at 701 North DuPont Blvd, Milford, DE 19963. She can be reached at 302.725.3420. We wish her every success in her new role.

New Provider: We are excited to welcome Dr. Abhi Freyer to Pointe Primary Care. In mid-to-late February, Dr. Freyer will begin seeing Medicare and Medicaid patients. Upon completion of credentialing, he will be able to see other insurances.

Patient Care Transition: While we would love to continue serving all our patients, our current staffing levels may require patients to transition to new providers. To assist during this time, patients transitioning will continue to have prescriptions renewed for up to 6 months, or they can secure a new provider, whichever is first. For prescription refills, please have your pharmacist submit them electronically or leave a message on our prescription line—pharmacy submissions are processed more quickly.

Medical Record Transfers:
Our administrative team is here to assist with transferring your medical records to a new provider. To initiate this process, please complete and return a Release of Information form.

IMPORTANT NOTICE:

Scam Calls

It has recently come to our attention that scammers have been making calls to impersonate Pointe Primary employees. We are actively investigating the issue.

What We Know:

The scammers are using a caller ID that says “Pointe Primary Care” and are sometimes mimicking legitimate Pointe Primary Care phone numbers.

They will ask to speak to the patient, ask how you are doing, and inform you that your Primary Care Physician has directed them to call you.

Spotting a Scammer:

Real Pointe Primary Care employees will provide their names when calling you.

Be wary of any “Pointe Primary Care” calls you were not expecting or seem out of the blue.  It is unlikely Pointe Primary Care would be contacting you except for:

• Scheduling and appointment reminders

• Follow-up about your care such as discussing test results

If you are unsure if the caller is a real Pointe Primary Care, do not provide any information. Our staff are aware of the situation and will not be offended – we would rather you err on the side of caution!

You can call Pointe Primary Care directly at 302.684.2000 to verify whether a call legitimately came from Pointe Primary Care.

Pointe Primary Care will NEVER try to sell you products or medical supplies.
